A hair specimen of 1.5 inches, about 200 strands (similar in thickness to a #2 pencil), cut closest to the scalp provides 100mg of hair, which is the optimal sample size for screening and confirmation purposes. For EtG tests, additional panels, or tests exceeding 10 types, a specimen weighing 150mg is advisable. It's best to measure the sample using a jeweler’s scale. If no scalp hair is available, a comparable quantity of body hair can be utilized. When we mention head hair, we mean scalp hair exclusively. Body hair is inclusive of all other hair types (such as facial hair, axillary hair, etc.).

Process Overview

The laboratory processing of a drug test result comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning begins with the incorporation of a sample into a laboratory system, verifying proper sealing and shipment of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data entry not available via an electronic chain of custody system.

Screening is a preliminary rapid assessment for drug abuse. While Screening offers a cost-efficient means of eliminating drug consumption in most samples, a positive screening result necessitates confirmation to be valid for legal proceedings. Samples showing presumptive positivity in Screening require a subsequent confirmation test.

Should a sample be presumptively positive during Screening, additional hair is retrieved from the original sample and prepared for Extraction. In this phase, drug compounds are extracted from hair at significantly lower concentrations than in alternative methodologies (such as urine or oral fluid), making hair drug screening one of the most challenging methods to execute.

Confirmation of any positive screening is performed using technologies like GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are appropriately washed before confirmation. Each step from Accessioning to Confirmation undergoes review in accordance with both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
