A 1.5-inch portion comprising around 200 hair strands (akin to the width of a #2 pencil) nearest the scalp yields 100mg of hair, which is the preferred sample for analysis and verification. For EtG, supplemental, and/or examinations exceeding 10 panels, a 150mg sample is advised. It's advisable to weigh the sample using a jeweler’s scale. If scalp hair is absent, an equivalent quantity of body hair can be collected. By head hair, we mean scalp hair alone. Body hair encompasses all other hair forms (facial, axillary, etc.).

Process Overview

The four fundamental phases encompassed in the lab handling of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a lab’s framework. This covers confirming the sample's seal and correct dispatch, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data tasks absent in an electronic chain of custody mechanism.

Screening entails an initial brief evaluation for substances of misuse. Although Screening is an economical approach to exclude drug consumption for most samples, a positive screen requires authentication for court acceptability. Samples that are presumptively positive in Screening necessitate corroborative verification.

If a sample is tendentiously positive during the Screening phase, additional hair is extracted from the initial sample for Extraction preparation. In this phase, substances are extracted at a significantly lower concentration from hair compared to alternative methodologies (e.g., urine or oral fluid), establishing hair drug screenings as the most intricate methodology to execute.

Verification of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All conditionally positive samples are cleansed before confirmation as required. The full laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
