A hair sample measuring 1.5 inches, consisting of roughly 200 strands (comparable in size to a #2 pencil) closest to the scalp equals 100mg, the optimal quantity for testing. For EtG, additional tests, or panels over 10, 150mg of hair is suggested. It’s advisable to weigh the hair on a jeweler’s scale. If scalp hair isn't available, an equivalent amount of body hair can be used. "Head hair" specifically refers to scalp hair, while "body hair" covers all other types (such as facial, axillary, etc.).

Process Overview

There are four primary stages in laboratory drug testing: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially processing the sample into the lab’s systems. This step includes ensuring the sample was sealed and shipped properly, assigning a unique LAN (Laboratory Accessioning Number), and completing any missing electronic chain of custody data entry.

Screening performs a preliminary check for drug presence. While cost-effective for eliminating drug usage in most samples, any positive results in Screening must be confirmed to be legally valid. Samples showing presumptive positives in Screening need additional verification.

Should a sample test presumptively positive during Screening, more hair is extracted from the specimen for the Extraction process. In this stage, drugs are withdrawn from hair at much lower levels than other methods like urine or oral fluids, which is why hair drug testing is the most challenging method.

Confirmation of a positive Screening is carried out using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positives are cleansed before confirmation if necessary. The complete process, from Accessioning to Confirmation, is conducted under the CAP (College of American Pathologists) Hair guidelines and accords with ISO / IEC 17025 certification.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
