A sample of 1.5 inches consisting of approximately 200 hair strands (comparable to the diameter of a #2 pencil) nearest the scalp yields 100mg of hair, which is optimal for assessment and validation. For EtG, additional tests, and/or tests exceeding 10 panels, a specimen amount of 150mg is advisable. It is suggested to weigh the specimen using a jeweler’s scale. If head hair is not available, an equivalent quantity of body hair may be used. When we mention head hair, this refers specifically to scalp hair, whereas body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test follows four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial incorporation of a specimen into a lab’s system. This step includes confirming the proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.

Screening involves an initial rapid evaluation for drugs of abuse. While Screening is an economical method to dismiss drug usage in most samples, a positive result in the screening phase requires confirmation to be valid in legal proceedings. Any samples that yield a presumptive positive in Screening require further confirmation.

Should a sample be presumptively positive during Screening, more hair is extracted from the original specimen and readied for Extraction. In this process, drugs are extracted from hair at a significantly lower concentration than in other methods (e.g., urine or oral fluid), making hair drug analysis the most complex method to execute.

Confirmation of any positive results identified in the screening phase is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleaned before confirmation as necessary. The entire lab process from Accessioning to Confirmation is assessed according to both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.
