For tests like EtG, additional panels, or those exceeding 10, 150mg of hair is advised. The specimen should be measured using a jeweler's scale. If scalp hair isn't available, an equivalent volume of body hair can be collected. Scalp hair refers solely to the head's hair, while body hair pertains to other types of hair (such as facial and axillary hair).

Process Overview

There are four primary steps in processing a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is entered into a lab's system. This process ensures the sample is correctly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and fills in missing information not included in an electronic chain of custody.

Screening serves as a preliminary rapid assessment for drug abuse. Although Screening is a cost-efficient method to exclude drug usage in most cases, any positive screening must be confirmed for it to be legally valid. Samples that test positive in Screening need a further confirmation.

Should a sample test positive during Screening, additional hair is taken from the original specimen for the Extraction phase. At this stage, drugs are extracted from the hair, although at lower concentrations than in other testing methods (like urine or oral fluid), which makes hair drug testing more challenging.

Any positive result from screening undergoes Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. Samples with a presumptive positive result are washed before confirmation as necessary. The entire process from Accessioning to Confirmation adheres to CAP (College of American Pathologists) Hair protocols and complies with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
