What it is, How it works

When acquired near the scalp, hair can offer a detection window of up to around 3 months for alcohol and other drugs. Hair collection is straightforward, relatively resistant to tampering, and convenient for shipping.

A 1.5-inch segment comprised of roughly 200 hair strands (approximately the width of a #2 pencil) nearest to the scalp will yield 100mg of hair, considered the optimal sample for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is advisable. We suggest weighing the specimen using a jeweler’s scale. In the absence of scalp hair, an equivalent quantity of body hair is acceptable. When we mention head hair, we are specifically referring to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The four primary phases in processing a drug test result in the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a sample within a laboratory’s system. This process includes checking that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any supplemental data entry not covered by an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drugs of abuse. While Screening is a cost-efficient approach to eliminate drug usage for the majority of samples, a positive screen requires confirmation to be court-admissible. Any samples that are presumptively positive during Screening necessitate a secondary confirmation.

If a sample tests presumptively positive at the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. Drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging methodology to execute.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo necessary washing before confirmation. The comprehensive laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
