A sample measuring 1.5 inches with roughly 200 hair strands (about equivalent to a #2 pencil) taken closest to the scalp provides 100mg of hair, the optimal quantity for both screening and confirmation. For EtG, add-ons, and/or analyses beyond 10 panels, it is advisable to use 150mg of the specimen. We suggest utilizing a jeweler’s scale for weighing the sample. If scalp hair is unavailable, an equivalent amount of body hair may be gathered. When we refer to head hair, we mean scalp hair specifically. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

There are four primary stages in the laboratory processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a laboratory’s system. This involves confirming that the sample was appropriately sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data not provided by an electronic chain of custody system.

Screening comprises an initial swift review for drugs of abuse. Although Screening is an economical way to eliminate drug use in most samples, a positive screen must be confirmed to be court-admissible. Any samples indicated as presumptively positive in Screening necessitate secondary confirmation.

If a sample is flagged as presumptively positive during Screening, additional hair is taken from the initial sample and prepared for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug testing is regarded as a complex procedure to conduct.

Positive screening results must be confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation if necessary. The complete laboratory procedure from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
