For EtG, additional tests, and/or panels exceeding ten, a specimen of 150mg is advisable. It's suggested to measure the sample with a jeweler’s scale. Should scalp hair not be accessible, a similar amount of body hair can be acquired. When indicating head hair, we are specifically referring to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The primary phases in the lab processing of a drug analysis result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample within a laboratory's system. This step includes ensuring the sample's seal and shipment integrity, assigning a LAN (Laboratory Accessioning Number) randomly, and completing any supplementary data input not provided by an electronic chain of custody system.

Screening serves as an initial rapid assessment for substance abuse. Screening offers a cost-efficient method to eliminate probable drug use in most samples, but a positive outcome requires confirmation for legal admissibility. Any samples deemed presumptively positive during Screening require further confirmation.

If a sample tests presumptively positive during the Screening phase, additional hair is extracted from the original specimen and prepped for Extraction. During this phase, drugs are isolated from hair at significantly lower concentrations than with other techniques (such as urine or oral fluid), emphasizing why hair drug analysis is the most complex testing methodology.

Any positive findings from screening are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS methods. Presumptive positive samples undergo washing before necessary confirmation. The complete lab process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
