A sample measuring 1.5 inches, consisting of roughly 200 hair strands (comparable to a #2 pencil's width) nearest to the scalp yields 100mg of hair, ideal for examination and verification. For EtG, additional tests, and those extending beyond 10 panels, 150mg of hair is advisable. Measurement of the sample using a jeweler's scale is suggested. Should scalp hair be unavailable, equivalent body hair may be utilized. "Head hair" specifically refers to "scalp hair," whereas "body hair" covers other hair varieties (like facial and axillary hair).

Process Overview

The primary stages in the lab processing of drug test outcomes include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is integrated into a lab's system, confirming that it was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and finishing any necessary data entry not covered by an electronic chain of custody.

Screening serves as a preliminary assessment to check for substance abuse. Although Screening is economical for excluding drug use in most samples, any positive findings need verification to be legally permissible. Presumptive positives in Screening require further confirmation.

If a sample tests presumptive positive during Screening, additional hair is extracted from the original sample for the Extraction process. Positive findings from Screening are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. Necessary cleansing of all presumptive positives is performed before confirmation. The complete process from Accessioning to Confirmation is scrutinized under both CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
