A 1.5-inch segment containing roughly 200 hairs (equivalent to the diameter of a #2 pencil) taken closest to the scalp will yield 100mg of hair, the optimal volume for initial screening and verification. For EtG, supplementary, and/or tests exceeding 10 panels, the recommendation is 150mg of hair. We advise using a jeweler's scale to measure the sample. If scalp hair is absent, an equivalent amount of body hair is acceptable. By head hair, we mean only scalp hair. Body hair encompasses all other types (e.g., facial, axillary).

Process Overview

The laboratory processing of a drug test result involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the start of a sample's journey into a lab's system. This step ensures the sample was properly sealed and sent, assigns a unique Laboratory Accessioning Number (LAN), and completes any necessary data entry not covered by an electronic chain of custody system.

Screening is an initial, quick assessment for substances of abuse. While Screening is a cost-efficient method to determine drug absence in most samples, a positive result requires verification to be admissible in legal scenarios. Any samples that test positive in Screening need a secondary confirmation.

If a sample tests positive in the Screening phase, additional hair from the original collection is extracted for analysis. During the Extraction phase, drugs are extracted from hair at much lower levels than other methods (e.g., urine or saliva), illustrating why hair drug analysis is considered complex to execute.

Confirmation of any positive results from screening is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples are washed as necessary. The comprehensive lab procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
