What it is, How it works

Hair analysis is increasingly recognized as an effective method for detecting alcohol and drug consumption. The fibers of a hair strand capture biomarkers, providing an extended overview of alcohol and drug history. Hair samples taken near the scalp can reveal alcohol and drug use for up to roughly three months. Collection is straightforward, tampering is relatively challenging, and shipping is uncomplicated.

A sample consisting of 1.5 inches of about 200 hair strands (approximate size of a #2 pencil) near the scalp yields around 100mg, considered the optimal quantity for screening and confirmation. For EtG, additional tests, or when conducting tests beyond 10 panels, a specimen of 150mg is advised. We suggest measuring the sample using a jeweler’s scale. If scalp hair is not an option, an equivalent quantity of body hair is a suitable alternative. When we mention head hair, we specifically refer to scalp hair. Body hair includes all other types such as facial or axillary hair.

Process Overview

The laboratory processing of a drug test result involves Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial integration of a sample into a lab’s system. It involves ensuring the sample was properly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.

Screening is a preliminary check for drugs of abuse. Although Screening offers a cost-effective means of identifying drug use in most samples, a positive screen must be confirmed for it to be legally valid. Any samples that test as presumptively positive in Screening need further confirmation.

Should a sample test as presumptively positive during Screening, more hair is extracted from the original sample for the Extraction stage. Here, drugs are retrieved from hair at significantly lower concentrations than other methods (such as urine or oral fluid), which is why hair drug screening is regarded as the most complex method.

Confirmation of any positive result from screening is achieved using GC/MS, GC/MS/MS or LC/MS/MS technologies. All samples that are presumptively positive are cleansed before confirmation if necessary. From Accessioning to Confirmation, the process is supervised in alignment with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
