When collected near the scalp, hair can offer a detection window of up to about 3 months for both alcohol and drugs. Hair collection is straightforward, moderately resistant to tampering, and convenient to transport.

An ideal sample involves a 1.5-inch section comprising roughly 200 hair strands (approximately equivalent to a #2 pencil in size) closest to the scalp, providing 100mg of hair for effective screening and verification. For EtG, additional tests, or panels exceeding 10, a 150mg sample is advised. Using a jeweler’s scale to weigh the specimen is recommended. In cases where scalp hair is absent, a comparable amount of body hair can be collected. By head hair, we mean only scalp hair, whereas body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

There are four principal steps in the laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the preliminary processing of a sample into the laboratory's system. It involves confirming the sample is properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not recorded by an electronic chain of custody system.

Screening entails an initial rapid assessment for substances of abuse. Though Screening is a cost-effective approach to eliminate drug usage for the majority of samples, a positive screen requires confirmation to be legally admissible. Samples showing presumptive positivity during Screening necessitate further confirmation.

When a sample tests presumptively positive during Screening, additional hair is extracted from the original specimen for the Extraction phase. Drugs are extracted from hair at concentrations much lower than in other methods (i.e., urine or oral fluid), hence hair drug testing is the most intricate methodology to execute.

Confirmation of any positive screening outcome is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as necessary. The entire laboratory procedure from Accessioning to Confirmation is supervised under both the CAP (College of American Pathologists) Hair category and the accreditation according to ISO / IEC 17025 standards.
