Process Overview

The laboratory process for a drug test result encompasses four primary phases: Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, the sample is initially integrated into the laboratory’s processing system. This involves ensuring the sample’s seal and shipment integrity, assigning a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data input absent in an electronic chain of custody system.

Screening involves a preliminary swift check for illicit substances. Although Screening is economical for excluding drug presence in most samples, a positive result must be validated to be permissible legally. Samples indicating a presumptive positive in Screening necessitate a subsequent validation.

Presumptive positives identified in the Screening phase require more hair from the original sample for Extraction. Here, drugs are isolated from hair at a concentration notably lower than in other methods (like urine or oral fluid), which underscores the complexity of conducting hair drug screening.

Any positive outcome from Screening is confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. Necessary presumptive positive samples undergo cleaning before confirmation. The entire process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair categorization and the adherence to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
