For EtG tests, add-ons, and/or tests extending beyond 10 panels, it’s suggested to use 150mg of the hair sample. It's advised to measure the specimen's weight using a jeweler's scale. If scalp hair isn't accessible, an equivalent amount of body hair may be opted for. By head hair, we specifically mean scalp hair. Body hair encompasses any other forms of hair (such as facial or axillary).

Process Overview

The four principal stages in processing a drug test result at a lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step, involving the processing of a sample into the lab’s system. This step ensures the sample's container was properly sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and records any further information not gathered by an electronic chain of custody.

Screening offers a preliminary assessment for substance abuse. While it is budget-friendly for eliminating drug use for most samples, any positive outcomes in Screening need further confirmation for admissibility in court. Presumptive positives in Screening necessitate secondary confirmation.

For samples presumptively positive post-Screening, additional hair is removed from the original sample for the Extraction process. At this stage, drugs are drawn out from hair in much lower concentrations compared to urine or oral fluid analysis, highlighting the complexity of hair drug testing.

Any positive findings from screenings undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS methods. Presumptive positive specimens are cleansed before confirmation as required. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
