For EtG, additional assessments, and/or tests exceeding 10 panels, 150mg of the sample is advisable. We suggest utilizing a jeweler's scale to weigh the sample. If scalp hair isn't obtainable, a similar quantity of body hair can be gathered. When we mention head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the first stage of integrating a sample into a laboratory's records. This step includes ensuring that the sample was suitably sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entries not facilitated by an electronic chain of custody system.

Screening is an initial swift evaluation for the presence of drugs. While Screening is efficient for eliminating drug presence in most samples, any positive screens require confirmation to be legally valid. Samples that exhibit a presumptive positive in Screening necessitate further confirmation.

If a sample shows a presumptively positive result during Screening, additional hair is extracted from the initial sample for the Extraction phase. In this step, drugs are isolated from hair at significantly lower concentrations than other methods (such as urine or oral fluid), which makes hair drug screening a challenging technique.

Confirmation of any positive screen result is performed using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The entire laboratory process, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
