For in-depth testing such as EtG, add-ons, and/or panels exceeding 10, it is suggested to collect 150mg. We advise using a jeweler's scale to weigh the specimen. Should scalp hair be unavailable, a comparable amount of body hair can be utilized. When discussing head hair, we mean scalp hair specifically. Body hair pertains to all other kinds such as facial or axillary hair.

Process Overview

Processing a drug test result in a lab entails four core steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary stage where a sample is entered into the lab system. This step involves confirming that the sample was securely sealed and shipped, assigning it a unique Laboratory Accessioning Number (LAN), and completing any outstanding data not covered by an electronic chain of custody system.

Screening serves as a preliminary examination for drug presence. While cost-effective for eliminating drug use in most samples, any positive result requires further confirmation to be legally valid. Samples that suggest the presence of drugs during Screening necessitate an additional verification step.

In cases where the Screening indicates a presumptive positive, additional hair from the initial sample is subjected to Extraction. This process removes drugs from hair at lower concentrations than other methods such as urine or oral fluid tests, highlighting the complexity of hair drug screening.

Confirming any positive Screening findings is done through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo a cleansing before confirmation as needed. The complete laboratory procedure from Accessioning to Confirmation is assessed according to both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 standards certification.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
