What it is, How it works

Utilizing hair testing has become recognized as an effective method for identifying drug and alcohol consumption. Hair reflects a prolonged record of alcohol and drug usage by capturing biomarkers within the strands of the growing hair. When obtained close to the scalp, it can offer roughly a three-month detection period for alcohol and drugs. Hair is straightforward to collect, relatively challenging to tamper with, and simple to transport.

A sample that is 1.5 inches long, consisting of about 200 hair strands (similar in size to a #2 pencil) near the scalp, provides 100mg of hair, the optimal amount for both screening and confirmation. For EtG, added tests, and/or tests exceeding 10 panels, 150mg of hair is advised. It is suggested to weigh the sample on a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair can be used. When referencing head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (such as facial or axillary hair).

Process Overview

There are four primary steps in processing a drug test result in the lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step in processing a sample within the lab’s system. This step ensures the sample was sealed and dispatched correctly, assigns it a random LAN (Laboratory Accessioning Number), and completes any extra data input absent from an electronic chain of custody system.

Screening is a preliminary rapid check for substances of abuse. While it's a cost-efficient approach to identify drug presence in most samples, a positive screen must be verified to be legally valid. Samples that test positive in Screening need further confirmation.

If a sample tests positive during Screening, more hair is extracted from the sample and prepared for Extraction. During this stage, drugs are extracted from hair in much lower concentrations compared to other methods (such as urine or oral fluids), which makes hair drug screening exceptionally challenging to conduct.

Confirming any positive result from screening is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test positive are washed before confirmation as necessary. The full lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair standard and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
