For EtG, additional panels, and/or tests exceeding 10 panels, a 150mg specimen is advisable. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is not available, the same quantity of body hair can be gathered. Only scalp hair is considered head hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The lab processing for a drug test result includes four vital stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with the sample's entrance into the laboratory’s framework. This stage involves verifying proper sealing and shipping of the sample, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any extra data entry absent in an electronic chain of custody system.

The Screening step consists of an initial examination for drug abuse. While Screening is an affordable method for ruling out drug use in most samples, a positive outcome necessitates Confirmation for legal validity. Samples with a presumptively positive Screening result need further confirmation.

In cases where a sample is presumptively positive during the Screening stage, more hair is extracted from the original specimen for the Extraction phase. Here, drugs are extracted from the hair at lower concentrations compared to other methods (e.g., urine or oral fluid), hence hair drug screening is considered more challenging.

Confirmation for any positive result from Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples undergo washing before Confirmation as needed. The entire lab process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and accredited according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
