What is checked in a DOT Physical?

Medical Conditions That Can Fail a DOT Physical

- Blood Pressure & Heart Disease: Unmonitored hypertension (≥180/110 mmHg), a recent heart event, stroke, uncontrolled angina, or an implanted defibrillator lacking approval.
- Diabetes: Unmanaged diabetes with recurrent episodes of low blood sugar or complications (neuropathy, visual issues) hindering safe driving.
- Sleep Apnea & Respiratory Issues: Neglected obstructive sleep apnea leading to daytime fatigue, or critical lung disease affecting oxygen levels.
- Neurological Disorders: Epilepsy/seizure conditions (unless exempt), conditions triggering sudden unconsciousness, vertigo, or uncontrolled tremors.
- Substance Abuse: Current use of illicit drugs, alcohol dependency, or misuse of prescribed medications impairing driving ability.
- Psychiatric & Cognitive Disorders: Serious uncontrolled mental illness (e.g., schizophrenia, bipolar disorder with recent episodes) or cognitive deficits impacting judgment and reaction.
There exists differentiation between temporary and permanent disqualification. Certain issues might only temporarily disqualify you until medical clearance or documentation such as controlled blood pressure or diabetes management is provided. In contrast, untreated seizure disorders or implanted defibrillators are generally considered permanent disqualifiers under FMCSA guidelines.
