Medical Conditions That Can Fail a DOT Physical

• Vision & Hearing: Vision must be no worse than 20/40 per eye with correction, and must distinguish traffic signal colors. Hearing must detect a forced whisper at 5 feet.
• Blood Pressure & Heart Issues: Hypertension uncontrolled at above 180/110 mmHg, or recent cardiac events, including heart attack, stroke, or angina, may disqualify without clearance.
• Diabetes: Unmanaged diabetes leading to frequent hypoglycemic events or nerve and vision complications can bar safe driving.
• Sleep & Breathing Disorders: Untreated sleep apnea causing daytime sleepiness, or severe respiratory illnesses affecting oxygenation, are disqualifying.
• Neurological Conditions: Epilepsy or conditions provoking sudden unconsciousness or uncontrolled tremors generally disqualify unless exempted.
• Substance Concerns: Illegal drug use or alcohol dependence that prejudices safe driving are disqualifying.
• Mental & Cognitive Health: Severe uncontrolled mental health issues like untreated schizophrenia or bipolar disorder can affect driver eligibility under FMCSA standards.
