What it is, How it works

A sample of hair measuring 1.5 inches with around 200 strands (approximately the width of a #2 pencil) from the scalp will yield 100mg, which is the ideal sample amount for screening and confirmatory tests. For EtG, additional analysis, and/or tests exceeding 10 panels, it is suggested to use 150mg of the sampled hair. We suggest using a jeweler's scale for weighing the specimen. If scalp hair isn't accessible, a comparable volume of body hair can be used. "Head hair" pertains solely to scalp hair, while "body hair" denotes all other hair types (facial, axillary, etc.).

Process Overview

The primary steps in processing drug test outcomes in a laboratory include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial intake of a sample into the laboratory system. It includes checking that the sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and entering any remaining data not captured by an electronic chain of custody system.

Screening is a preliminary inspection for illegal drug use. Although Screening is cost-efficient for detecting drug abstinence in most samples, a positive screening result needs confirmatory testing for legal validity. Samples regarded as presumptively positive during Screening necessitate additional confirmation.

Should a sample test presumptively positive in Screening, additional hair is extracted from the initial sample for Extraction. During Extraction, drugs are retrieved from hair at a significantly lower concentration than with other methods (e.g., urine or oral fluid), which contributes to the challenge of hair drug screening methods.

Positive screening results are confirmed through methods such as GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation when necessary. The process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
