For EtG, additional tests, and/or tests exceeding 10 panels, a recommendation of 150mg of the specimen is set. It is advised to measure the sample weight using a jeweler's scale. In the event that scalp hair is not available, a comparable amount of body hair can be used. When discussing head hair, it pertains specifically to scalp hair. Body hair indicates all other types of hair (facial, axillary, etc.).

Process Overview

The primary phases of processing a drug test result in the lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the initial integration of a sample into a lab system. This includes ensuring the sample was appropriately sealed and dispatched, generating a random LAN (Laboratory Accessioning Number), and filling out any necessary data that wasn't supplied by an electronic chain of custody protocol.

Samples that are tentatively positive in Screening necessitate an additional confirmation step.

If the Screening phase indicates a potential positive outcome, supplementary hair is extracted from the original sample and slated for Extraction. In this phase, drugs are drawn out from the hair at a significantly reduced concentration compared to other methodologies (such as urine or oral fluid), which is what renders hair drug screening the most challenging methodology to conduct.

Confirmation of any positive screening result is executed by means of GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all samples presumed positive undergo a washing process as necessary. The complete laboratory procedure from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation along with accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
