All other hair, such as facial and axillary, is categorized as body hair.

Process Overview

The laboratory processing of a drug test outcome encompasses four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory's framework. This step involves ensuring the sample's proper sealing and shipment, assigning a unique LAN (Laboratory Accessioning Number), and fulfilling any remaining data entry requirements not covered by an electronic chain of custody system.

Screening provides a preliminary check for drugs of abuse. While Screening is a cost-effective method to eliminate drug use in most samples, a positive result requires confirmation to be valid in legal settings. Samples flagged as presumptively positive in Screening necessitate further verification.

If a sample is flagged in the Screening stage, additional hair is taken from the original specimen for the Extraction step. Here, drugs are isolated from hair at significantly reduced concentrations compared to other methods (such as urine or oral fluid), making hair drug testing particularly complex.

Confirmation of a presumptive positive from Screening is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All presumed positive samples undergo washing before confirmation as needed. The full laboratory procedure from Accessioning to Confirmation is conducted under the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
