For EtG, additional tests, and screens exceeding ten panels, a 150mg specimen is advised. Using a jeweler’s scale to weigh the specimen is recommended. In the absence of scalp hair, a corresponding amount of body hair can be used. References to head hair are specific to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The processing of a drug test in the laboratory consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of integrating a sample into a laboratory's system. It involves checking that the sample was correctly sealed and shipped, allocating a random LAN (Laboratory Accessioning Number), and entering any supplementary data that is not included in an electronic chain of custody system.

Screening consists of a preliminary assessment for drugs of abuse. While Screening offers a cost-effective way to rule out drug use in the majority of samples, a positive result requires confirmation for it to be legally permissible. Confirmation of any positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples showing presumptive positives undergo washing prior to confirmation as required. The comprehensive laboratory process, from Accessioning to Confirmation, undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
