For EtG enhancements and/or tests exceeding 10 panels, a collection of 150mg is advised. It is suggested to verify the weight of the specimen using a jeweler’s scale. Should scalp hair be unavailable, an equivalent quantity of body hair can be collected. References to head hair specifically mean scalp hair. In contrast, body hair encompasses all other types of hair, such as facial and axillary hair.

Process Overview

The laboratory processing of a drug test result typically includes four major steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample undergoes initial processing into a laboratory’s system. This phase ensures the sample is sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry required beyond what an electronic chain of custody system provides.

The Screening phase entails a preliminary examination for drugs of abuse. All presumptive positives in Screening necessitate secondary confirmation.

If a sample indicates a presumptive positive during Screening, additional hair from the original specimen undergoes preparation for Extraction. At this stage, drugs are extracted from hair in significantly lower concentrations than alternative techniques (e.g., urine or oral fluid), explaining why hair drug screening is notably challenging.

Confirmation of any positive screening outcome is achieved using GC/MS, GC/MS/MS or LC/MS/MS. All samples presumed positive undergo washing before confirmation as needed. The entire lab procedure from Accessioning to Confirmation is reviewed according to both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
