For EtG, extra tests, and panels exceeding 10, 150mg of hair is preferred. It’s advisable to weigh the sample using a jeweler’s scale. If scalp hair is not an option, a comparable amount of body hair may be used. References to head hair indicate only scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The testing process in laboratories encompasses four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample within a lab's system, ensuring it's sealed and sent correctly, allocating a random Laboratory Accessioning Number (LAN), and completing any data not included in an electronic chain of custody.

Screening is the initial step to identify potential substance abuse. Although it's a cost-efficient strategy to exclude drug consumption in most samples, a positive screening result requires confirmation for legal admissibility. Any samples that return a presumptive positive from Screening necessitate further confirmation.

Should a sample appear presumptively positive during Screening, more hair is sourced from the primary sample for Extraction. Within this phase, substances are isolated from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair analysis a challenging technique.

Positive Screening results are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All apparent positive samples undergo washing before confirmation if needed. The full testing cycle, from Accessioning to Confirmation, adheres to CAP (College of American Pathologists) Hair provisions and ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
