What it is, How it works

When cut near the scalp, hair can offer a detection period of up to around three months for drugs and alcohol. The collection process is straightforward, tampering is relatively challenging, and shipping is straightforward.

Gathering a 1.5-inch length sample of approximately 200 hair strands (comparable to the thickness of a #2 pencil) from the closest point to the scalp yields 100mg of hair, the preferred amount for both screening and verification. For EtG, additional tests, and/or assessments exceeding 10 panels, 150mg of the sample is advised. We advocate for the use of a jeweler's scale to weigh the sample. Should scalp hair be unavailable, an equivalent quantity of body hair is an acceptable alternative. The term scalp hair specifically refers to hair from the head, while body hair includes all other hair types (facial, underarm, etc.).

Process Overview

The laboratory assessment of a drug test outcome involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the initial stage of incorporating a sample into a lab's system. This phase encompasses ensuring that the sample was properly sealed and delivered, assigning an arbitrary LAN (Laboratory Accessioning Number), and finishing any supplementary data input absent from an electronic chain of custody setup.

Screening entails an initial rapid evaluation for drugs of misuse. Although Screening is an economical approach to eliminate drug consumption for most samples, a positive screen necessitates verification for legality in court. Any specimens with a preliminary positive Screening result need a follow-up confirmation.

Upon a presumptive positive outcome during the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. Within this phase, drugs are derived from hair at a significantly lower density compared to other techniques (e.g., urine or oral fluid), underscoring why hair drug testing remains the most intricate technique to execute.

Validating any positive screening finding occurs through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to validation, all presumptive positive samples undergo washing as necessary. The full laboratory procedure from Accessioning to Confirmation undergoes examination under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 certification standards.

The manuscripts in this section presented novel approaches, such as multianalyte methods and on-line preparation which can enable rapid analysis and improve thorough forensic investigations.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
