A 1.5-inch segment of about 200 strands (roughly the circumference of a #2 pencil) nearest to the scalp yields 100mg of hair, which is optimal for both screening and confirmation procedures. For EtG add-ons and/or tests exceeding 10 panels, a 150mg specimen is advisable. It is recommended to use a jeweler’s scale for weighing the specimen. If scalp hair is missing, an equal quantity of body hair might be utilized. "Head hair" pertains solely to scalp hair, whereas "body hair" refers to all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory’s database. This process includes ensuring the sample's seal and proper shipping, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data entry that isn't covered by an electronic chain of custody system.

Screening is a preliminary examination for substances of abuse. Although Screening offers a cost-efficient method to rule out drug use in most samples, a positive screen requires confirmation for legal validity. Any samples hinting at positivity in Screening necessitate further confirmation.

Should a sample indicate positivity during the Screening phase, additional hair is extracted from the original sample for the Extraction process. At this stage, drugs are isolated from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), making hair drug screening one of the most challenging procedures.

GC/MS, GC/MS/MS, or LC/MS/MS is employed to confirm any positive Screening result. Presumptive positive samples undergo washing before confirmation if required. The comprehensive laboratory journey from Accessioning to Confirmation is monitored under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
