What it is, How it works

Hair analysis has emerged as a robust method for identifying drug and alcohol use. Hair can capture a comprehensive historical record of alcohol and drug consumption by embedding biomarkers within the fibers of the hair as it grows. When collected near the scalp, hair can offer a detection period of up to roughly 3 months for alcohol and drugs. Hair is straightforward to gather, relatively resistant to tampering, and simple to transport.

A 1.5-inch sample comprising roughly 200 hair strands (about the width of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal amount for analysis and validation. For EtG, additional analyses, and/or tests exceeding 10 panels, a 150mg sample is advised. We propose using a jeweler's scale for weighing the sample. If scalp hair is unobtainable, a proportional quantity of body hair may be utilized. By head hair, we specifically mean scalp hair. Body hair includes all other varieties (such as facial, axillary, etc.).

Process Overview

The procedure comprising the lab processing of a drug test outcome entails Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample into the lab's system. This process consists of checking that the sample was properly sealed and sent, assigning a unique LAN (Laboratory Accessioning Number), and fulfilling any additional data input absent an electronic chain of custody mechanism.

Screening offers a preliminary rapid examination for substances of misuse. While Screening is an economical method to exclude drug use across most specimens, a positive screen must be verified for legal admissibility. Any samples tentatively positive in Screening necessitate secondary confirmation.

If a sample tests tentatively positive in the Screening phase, extra hair is extracted from the original sample and readied for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening findings is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All tentatively positive samples undergo washing before confirmation. The entire lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

Note: Although often termed as "hair follicle tests", the analysis examines the hair strand rather than the follicle beneath the scalp

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
