What it is, How it works

Hair testing has emerged as a potent method for identifying drug and alcohol consumption. Hair offers an extended historical account of substance use by embedding biomarkers within the fibers of its developing strand. When cut near the scalp, hair can reveal a detection timeframe reaching up to roughly three months for alcohol and other substances. Collecting hair is straightforward, challenging to tamper with, and convenient for shipping.

A 1.5-inch section consisting of approximately 200 hair strands (roughly the size of a #2 pencil) nearest the scalp will yield 100mg of hair, the perfect sample for analysis. For EtG additions and/or assessments over 10 panels, procuring 150mg of the hair is advisable. It's suggested to use a jeweler’s scale for weighing the sample. If scalp hair is unavailable, a similar quantity of body hair can be used. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four pivotal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample's initial integration into the lab’s system. This step verifies the sample's proper sealing and shipment, assigns a unique LAN (Laboratory Accessioning Number), and completes any missing data entry not captured by an electronic chain of custody system.

Screening is a preliminary assessment for drug presence. While it's cost-efficient for disqualifying drug use in the majority of samples, a positive screening must be confirmed to be acceptable in legal contexts. Samples that test positive on the Screening require further confirmation.

Should a sample test positive during Screening, additional hair is taken from the original sample and readied for Extraction. Drugs are extracted from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), thus making hair drug screening a challenging procedure.

Confirmation of any positive screening outcome is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptively positive are cleaned prior to confirmation if necessary. The complete lab process, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
