For examinations involving EtG, additional substances, or beyond the 10-panel spectrum, 150mg of hair is preferred. Utilize a jeweler’s scale for precise measurement. If scalp hair cannot be obtained, an equivalent quantity of body hair might be used. By head hair, we mean scalp hair specifically. All other hair types (facial, axillary, etc.) are classified generally as body hair.

Process Overview

The primary stages in processing a drug test in a lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to initially handling the sample within a laboratory. This comprises confirming the sample's securement and shipment, attaching a random LAN (Laboratory Accessioning Number), and completing additional data input absent from an electronic custody tracking system.

Screening offers a preliminary evaluation for substances. Although Screening is cost-efficient for excluding drug presence in most samples, any non-negative result necessitates validation before court approval. Presumptive positives from Screening require further confirmation.

Once a sample tests non-negative during Screening, additional hair is retrieved from the initial sample for Extraction. In this process, drugs are taken from hair in lower concentrations compared to methods like urine or saliva testing, contributing to the complexity of hair analysis.

Positive screening results undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to Confirmation, presumptive positives are cleansed as required. The entire process from Accessioning to Confirmation is scrutinized in accordance with CAP (College of American Pathologists) Hair specifications and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
