When extracted near the scalp, hair can reveal a usage timeline of up to roughly 90 days for alcohol and other substances. Hair is straightforward to collect, relatively resistant to tampering, and simple to transport.

A sample measuring 1.5 inches, consisting of 200 hair strands (approximately the thickness of a #2 pencil) nearest the scalp, constitutes a 100mg sample, which is optimal for testing and confirmation. For EtG, additional panels, and/or tests exceeding 10 panels, a 150mg sample is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is not available, a comparable quantity of body hair can be used. When we mention head hair, we specifically mean scalp hair, while body hair includes all other types (e.g., facial, axillary).

Process Overview

The primary stages in laboratory processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into a laboratory’s procedures. This involves confirming that the sample was appropriately sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening is a preliminary quick assessment for substance abuse. Though Screening cost-effectively excludes drug usage in most samples, a positive screening must be validated for court admissibility. Samples that test positive in Screening require follow-up confirmation.

Should a sample test positive in Screening, additional hair from the original sample is extracted for Extraction, where drugs are isolated from hair at a much lower concentration than other methods (e.g., urine or oral fluid), making hair drug tests the most challenging methodology.

Confirmation of any positive screening result is performed with GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples are cleansed before confirmation as necessary. The entire lab process, from Accessioning to Confirmation, is audited under both the CAP (College of American Pathologists) Hair classification and ISO/IEC 17025 accreditation.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
