For EtG, additional testing, and/or examinations exceeding 10 panels, 150mg of the specimen is advised. It is suggested to measure the specimen using a jeweler’s scale. If head hair is not available, a comparable amount of body hair may be used. When mentioning head hair, it pertains to scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a laboratory’s system. This step ensures the sample was sealed and transported correctly, assigns a random LAN (Laboratory Accessioning Number), and fulfills any additional data entry absent from an electronic chain of custody system.

Screening is a preliminary rapid check for drugs of abuse. Though Screening is a cost-effective approach to eliminating drug usage in most samples, a positive screening necessitates confirmation for court admissibility. Any samples indicating presumptive positivity in Screening require a subsequent confirmation.

When a sample shows presumptive positivity at the Screening stage, further hair is extracted from the initial specimen and prepared for Extraction. In this phase, drugs are separated from hair at significantly lower concentrations than other methods (such as urine or oral fluid), making hair drug screening one of the most challenging methodologies to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples are washed as necessary. The laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 benchmarks.
