What it is, How it works

Hair testing is acknowledged as an effective method for identifying drug and alcohol usage. Hair captures a lengthy record of alcohol and other substances by embedding biomarkers within the threads of the developing hair strand. When obtained near the scalp, hair can reveal a detection span of up to roughly three months for both alcohol and other drugs. Hair is straightforward to collect, somewhat resistant to tampering, and easy to transport.

A 1.5-inch portion comprising approximately 200 strands of hair (around the dimension of a #2 pencil) nearest to the scalp yields 100mg of hair, considered the optimal sample for evaluation and verification. For EtG, add-ons, and/or screenings beyond 10 panels, 150mg of the sample is advised. It's recommended to measure the specimen using a jeweler’s scale. Should scalp hair be inaccessible, an equivalent amount of body hair may be harvested. The term head hair specifically refers to scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the initial introduction of a sample into a laboratory’s records. This process entails confirming that the sample was sealed and sent correctly, generating a random LAN (Laboratory Accessioning Number), and filling in any supplementary data not entered via an electronic chain of custody system.

Screening is a preliminary swift examination for drugs of abuse. Although Screening is an economical approach to dismiss drug use in most samples, a positive screen must be verified for legal acceptability. Any samples that are presumptively positive in Screening do require a secondary confirmation.

If a sample is presumptively positive in the Screening stage, more hair is retrieved from the original sample and readied for Extraction. In this phase, drugs are isolated from hair at a much finer concentration compared to other approaches (e.g., urine or oral fluid), which is why hair drug screening presents the most challenging methodology to execute.

Verification of any positive screening outcome is accomplished through GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as necessary. The entire laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Long detection window: Hair drug tests can detect drug use for up to 90 days, compared to urine tests which have a shorter detection window.
  • Difficulty to cheat: It is very difficult to cheat a hair drug test, making the results more accurate.
  • Provides historical data: It can provide a pattern of drug use over a period of time, not just recent use.

Limitations:

  • Cannot detect recent use: It takes approximately 5-7 days for drugs to be detectable in hair.
  • Cost: Hair drug tests are generally more expensive than other drug testing methods.
  • Variations in results: Factors such as hair color and individual variations in hair growth can affect the concentration of drug metabolites in the hair.

Note: Despite frequently being termed "hair follicle tests", the test itself examines the hair strand and not the hair follicle beneath the scalp
