For EtG, add-ons, and/or screenings beyond 10 panels, 150mg of the sample is advised. It's recommended to measure the specimen using a jeweler’s scale. Should scalp hair be inaccessible, an equivalent amount of body hair may be harvested. The term head hair specifically refers to scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the initial introduction of a sample into a laboratory’s records. This process entails confirming that the sample was sealed and sent correctly, generating a random LAN (Laboratory Accessioning Number), and filling in any supplementary data not entered via an electronic chain of custody system.

Screening is a preliminary swift examination for drugs of abuse. Although Screening is an economical approach to dismiss drug use in most samples, a positive screen must be verified for legal acceptability. Any samples that are presumptively positive in Screening do require a secondary confirmation.

If a sample is presumptively positive in the Screening stage, more hair is retrieved from the original sample and readied for Extraction. In this phase, drugs are isolated from hair at a much finer concentration compared to other approaches (e.g., urine or oral fluid), which is why hair drug screening presents the most challenging methodology to execute.

Verification of any positive screening outcome is accomplished through GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as necessary. The entire laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
