A 1.5-inch segment of about 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp provides 100mg of hair, the perfect sample size for analysis and verification. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg is advised. We suggest using a jeweler’s scale to measure the sample. If head hair is lacking, an equivalent quantity of body hair can be used. When mentioning head hair, we refer exclusively to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the step where a sample is initially processed into the lab’s framework. This step encompasses confirming that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent from an electronic chain of custody protocol.

Screening provides a preliminary rapid assessment for substance misuse. Although Screening is an affordable method to dismiss drug use in most samples, a positive outcome from screening must be verified for court admissibility. Any samples that appear positive in Screening require another verification.

Should a sample be positive in Screening, additional hair is collected from the original specimen and prepared for Extraction. During this phase, drugs are separated from hair at much lower levels compared to other techniques (e.g. urine or oral fluid), making hair drug testing one of the most challenging methods to execute.

Any positive Screening result is confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All potentially positive samples are cleansed before confirmation as necessary. The comprehensive lab process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and the certification to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
