Hair tests offer a significant advantage, providing a detection window of up to 90 days, conducted in observed procedures, and proving very challenging to falsify. Yet, urine tests remain more precise for detecting recent drug use within 5 days.

What it is, How it works

The practice of hair testing is increasingly recognized as an efficient method for detecting drug and alcohol consumption. By capturing biomarkers within the strands, hair offers a detailed historical record of substance use. When collected from near the scalp, this method allows for a detection timeframe extending up to around 90 days. The collection process for hair samples is straightforward, with a lesser risk of tampering, and the samples are easy to transport.

For optimal analysis, a 1.5-inch segment of approximately 200 hair strands (similar in thickness to a #2 pencil) near the scalp provides 100mg, the preferred sample size for both initial and confirmatory evaluations. For EtG, supplemental tests, and/or panels exceeding 10, a 150mg sample is suggested. It's advisable to use a jeweler’s scale for weighing. In the absence of scalp hair, a comparable volume of body hair can be used. The term "head hair" specifically denotes scalp hair. "Body hair" encompasses all other hair types, including facial and axillary hair.

Overview of the Procedure

The drug testing process in a laboratory comprises four essential phases: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, samples are initially integrated into a laboratory’s workflow, which involves checking the proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any data entry not included in an electronic chain of custody system.

Screening involves an initial, rapid assessment for the presence of drugs. While Screening is an economical method to exclude drug use in many samples, legally acceptable positive findings need confirmation. Presumptively positive samples identified during Screening need a secondary verification step.

Should a sample show a presumptive positive result during Screening, additional hair is extracted from the original sample for the Extraction phase. In this step, drugs are isolated from the hair at concentrations much lower than those found in other testing methods (such as urine or oral fluid), highlighting why hair drug screening presents significant challenges.

Positive screenings are subsequently confirmed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Prior to confirmation, all samples showing presumptive positives undergo washing as necessary. The entire lab process from Accessioning to Confirmation is conducted within the scope of both CAP (College of American Pathologists) Hair guidelines and accreditation to ISO / IEC 17025 standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
