What it is, How it works

The fibers of growing hair strands encapsulate biomarkers, offering an extended history of alcohol and drugs. Collecting hair close to the scalp enables a detection window up to roughly three months. Hair collection is straightforward, hard to tamper with, and easy to transport.

A sample comprising a 1.5-inch section of about 200 hair strands (comparable to the size of a #2 pencil) near the scalp results in 100mg of hair, which is the perfect sample size for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is suggested. Utilizing a jeweler’s scale for weighing the sample is recommended. In the absence of scalp hair, an equivalent amount of body hair could be used. In this context, head hair refers exclusively to scalp hair. Body hair refers to all other types of hair (e.g., facial, axillary).

Process Overview

The key stages in processing a drug test in the laboratory are Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the primary handling of the sample into the laboratory’s framework. This step includes verifying the sample’s sealing and shipping status, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent in an electronic chain of custody system.

With Screening, there’s an initial fast check for drugs. Though cost-effective for eliminating drug usage in most samples, a positive screen needs confirmation for court admissibility. Samples presumed positive during Screening require a secondary confirmation.

Should a sample indicate a presumptive positive during Screening, additional hair is extracted from the original specimen for the Extraction phase. In this phase, the drugs are extracted from hair at significantly lower concentrations compared to other techniques (e.g., urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation of any positive screening is achieved through GC/MS, GC/MS/MS, or LC/MS/MS methodologies. All samples presumed positive are washed before confirmation based on necessity. The whole laboratory method from Accessioning right through to Confirmation is rigorously analyzed under the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 accreditation.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
