What it is, How it works

When gathered nearer the scalp, it can provide detection for up to about 90 days for alcohol and other drugs. Hair is effortless to gather, relatively tough to manipulate, and convenient to ship.

An ideal 1.5-inch sample involves approximately 200 strands of hair (around the width of a #2 pencil) closest to the scalp, yielding 100 mg of hair, perfect for both screening and confirmation purposes. For EtG, additional tests, and/or panels beyond 10, a specimen weighing 150 mg is advisable. Measuring the specimen on a jeweler’s scale is recommended. If scalp hair is not available, a similar amount of body hair may be used. When referencing head hair, we mean scalp hair specifically. Body hair encompasses all other hair types like facial and axillary.

Process Overview

The laboratory processing of a drug test result encompasses four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial inputting of a sample into the lab’s system. This step includes ensuring the sample was correctly sealed and transported, assigning it a random LAN (Laboratory Accessioning Number), and finalizing any additional data input absent in an electronic chain of custody system.

Screening offers a rapid initial check for drugs of abuse. It’s an economical approach to eliminate drug usage in most samples; however, a positive screen requires confirmation for legal admissibility. Any samples presumptively marked as positive during Screening warrant further confirmation.

If presumed positive at the Screening phase, more hair is extracted from the original specimen in preparation for Extraction. At this step, drugs are derived from hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), which explains the complexity of hair drug screening.

GC/MS, GC/MS/MS, or LC/MS/MS methods conduct confirmation of any positive screening result. Before confirmation, all presumptive positive samples are washed as needed. The full laboratory procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
