What it is, How it works

Hair captures a long-term record of alcohol and drug presence by securing biomarkers within the fibers of the hair strand as it grows. When collected near the scalp, hair offers up to a 3-month detection period for alcohol and drugs. Hair collection is straightforward, relatively challenging to tamper with, and easy to transport.

A 1.5-inch section containing approximately 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, the preferred quantity for screening and validation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is advised. It is beneficial to measure the sample using a jeweler’s scale. If scalp hair is not available, a matching amount of body hair can be gathered. References to head hair specifically mean scalp hair. Body hair pertains to all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial step of processing a sample into a laboratory system. This involves confirming that the sample was correctly sealed and shipped, allocating a random LAN (Laboratory Accessioning Number), and completing any additional data entry not included in an electronic chain of custody framework.

Screening provides an initial rapid assessment for drugs of abuse. Although Screening efficiently excludes drug usage in the majority of samples, a positive screen needs confirmation to be courtroom admissible. Any samples that test presumptively positive during Screening require secondary confirmation.

If Screening suggests a positive result, additional hair is taken from the initial sample and readied for Extraction. During this phase, drugs are extracted from hair at considerably lower concentrations compared to other methods (e.g., urine or oral fluid), thus making hair drug testing a challenging methodology.

Confirmation for any positive Screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed as required before confirmation. The entire laboratory process, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
