If scalp hair is unavailable, equivalent body hair can be taken. "Head hair" refers specifically to scalp hair, while "body hair" encompasses all other types (like facial or axillary hair).

Process Overview

The lab processing of a drug test involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with logging a sample into the lab’s system, ensuring the sample’s seal and shipment are intact, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry absent in the electronic chain of custody system.

Screening includes a preliminary inspection for drugs of abuse. While cost-effective for ruling out drug usage in most samples, a positive screen must be confirmed to be court-admissible. Any specimens that test presumptive positive during Screening need further confirmation.

Samples testing presumptive positive in Screening have additional hair extracted from the specimen for the Extraction process. Here, drugs are extracted from hair at much lower concentrations than with other methods (e.g., urine or oral fluid), which is why hair drug screening is notably challenging to perform.

Confirmation of a positive screen is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples are cleansed as needed. The full process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
