A sample of about 200 strands, measuring 1.5 inches from the scalp (similar to a #2 pencil), yields 100mg of hair, ideal for both preliminary and confirmatory testing. For EtG, additional tests, or panels beyond 10, a sample of 150mg is advisable. It is best to weigh the hair on a jeweler’s scale. If scalp hair is absent, an equivalent amount of body hair can be taken. By head hair, we mean scalp hair specifically. Body hair encompasses facial, axillary, etc.

Process Overview

The drug testing process in the lab includes four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to processing the sample into the laboratory system initially. This step ensures the sample was sealed and sent correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry omitted by an electronic chain of custody system.

Screening is a preliminary review for illicit drugs. Though economically efficient for excluding drug use in most samples, those that test positive in Screening need confirmation for court admissibility. Samples showing a presumptive positive in Screening demand additional confirmation.

For a presumptively positive sample during the Screening phase, more hair is extracted from the initial sample for Extraction. This step extracts drugs at lower concentration levels compared to other methods (e.g., urine or oral fluid), highlighting why hair drug testing is particularly complex.

Confirmation of positive screening outcomes is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as required. The complete process, from Accessioning through Confirmation, is audited under both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
