What it is, How it works

When obtained near the scalp, hair can reveal substance use up to a duration of roughly three months. The collection process is straightforward, hard to tamper with, and convenient to transport.

A 1.5-inch segment of around 200 hair strands (resembling a #2 pencil’s width) from the scalp yields 100mg, optimal for both preliminary testing and confirmation. For EtG, supplementary screens, or panels exceeding 10, a 150mg sample is advisable. We suggest utilizing a jeweler’s scale for measuring. In cases where scalp hair is not available, an equivalent amount of body hair may be used. When we mention head hair, it refers exclusively to scalp hair. Body hair includes all other varieties (facial, axillary, etc.).

Process Overview

The procedure for laboratory processing of a drug test consists of four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into the lab system. This process ensures proper sealing and shipping of the sample, assigns a unique LAN (Laboratory Accessioning Number), and completes any remaining data entry that an electronic chain of custody system might not provide.

Screening serves as an initial rapid assessment for substance presence. Although economically efficient in excluding drug usage for most samples, a positive screen must be verified to hold up in court. Any sample testing presumptively positive at this stage requires further confirmation.

Presumptively positive samples in the Screening phase involve extracting additional hair from the original sample for Extraction. During Extraction, drugs are isolated from the hair at much lower concentrations compared to other techniques (urine, oral fluid), making hair drug analysis notably challenging.

Confirmation of any positive screen is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Samples that are positive are cleansed before confirmation, as needed. The entire lab operation, from Accessioning to Confirmation, is subjected to a review under both the CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
