What it is, How it works

A 1.5-inch hair segment consisting of roughly 200 strands (around the diameter of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal amount for both screening and confirmation purposes. For EtG, additional analyses, and/or tests exceeding 10 panels, 150mg of hair is advised. We advise measuring the sample with a jeweler’s scale. If scalp hair is unattainable, a similar volume of body hair might be used. When we discuss head hair, it exclusively pertains to scalp hair. Body hair encompasses all other hair varieties (like facial, axillary, etc.).

Process Overview

The procedure in the lab for assessing a drug test result comprises four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary handling of a sample within a laboratory’s framework. This step encompasses examination to ensure the sample was properly sealed and transported, the assignment of a unique LAN (Laboratory Accessioning Number), and the fulfillment of any supplementary data requirements not provided through an electronic chain of custody mechanism.

Screening encompasses an initial rapid assessment for abused substances. Though Screening affords an economical solution to exclude drug use in a majority of samples, a positive result requires confirmation to be acknowledged in legal proceedings. Any specimens that register presumptively positive in Screening demand a subsequent verification.

Should a sample show presumptive positivity during the Screening, additional hair is extracted from the original sample and readied for Extraction. This phase involves the capturing of drugs from hair at significantly lower levels compared to other methods (e.g., urine or oral fluid), explaining why hair drug analysis is one of the most complex methods to execute.

Verification of any positive outcomes from screening is conducted using GC/MS, GC/MS/MS, or LC/MS/MS technology. Prior to confirmation, all presumptively positive samples undergo necessary washing. Each step in the laboratory workflow from Accessioning to Confirmation is validated under both the CAP (College of American Pathologists) Hair classification and accredited to ISO / IEC 17025 benchmarks.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
