A 1.5-inch section consisting of about 200 hair strands (approximately the thickness of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the preferred sample for testing and validation. For EtG, add-ons, and/or evaluations exceeding 10 panels, a 150mg sample is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair cannot be used, an equivalent amount of body hair may be taken. When we talk about head hair, it specifically refers to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory process for analyzing a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary handling of a sample into the lab's system. This entails ensuring the sample was properly sealed and transmitted, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entry not covered by an electronic chain of custody system.

Screening entails an initial, rapid assessment for controlled substances. While Screening is cost-effective for eliminating drug use in the majority of samples, a positive screen must be validated to hold up in court. Samples that test positive during Screening do require follow-up confirmation.

If a sample is positive at the Screening phase, additional hair is extracted from the initial sample for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging method to execute.

Validation of any positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All positive samples are washed if necessary prior to confirmation. The laboratory procedure from Accessioning to Confirmation is conducted under both the CAP (College of American Pathologists) Hair specification and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
