For EtG, additional inspections, and/or more comprehensive screenings over 10 categories, 150mg of the sample is suggested. We propose utilizing a jeweler’s scale for weighing. If head hair is not present, an equivalent amount of body hair can be used. When mentioning head hair, it specifically means scalp hair. Body hair is inclusive of all other hair types (facial, axillary, etc.).

Process Overview

There are four primary stages in the laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial incorporation of a sample into a laboratory's system. This procedure ensures that the sample was adequately sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and performs any additional data entry not covered by an electronic chain of custody system.

Screening is a preliminary quick assessment for drugs of abuse. While Screening is a cost-efficient method to exclude drug use in the majority of samples, a positive screen must be substantiated for court acceptance. Samples that test positive indicatively through Screening need secondary confirmation.

If a sample tests positive indicatively during Screening, additional hair from the original specimen is prepared for Extraction. At this stage, drugs are extracted from the hair at a considerably lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging methodology to carry out.

Confirmation of any positive Screen result is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed prior to confirmation as required. The complete laboratory procedure from Accessioning to Confirmation undergoes a review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
