What it is, How it works

Hair testing is recognized as a potent method for detecting drug and alcohol consumption. Hair offers a lengthy historical record of drug and alcohol use by capturing biomarkers within the fibers of the hair as it grows. When hairs are obtained near the scalp, they can offer a detection timeframe of up to around three months for alcohol and drugs. Hair samples are easily obtained, somewhat resistant to tampering, and convenient for transportation.

A 1.5-inch snippet consisting of roughly 200 hair strands (comparable to the girth of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is optimal for testing and verification. For EtG, extra tests, and/or evaluations exceeding 10 panels, a 150mg sample is advised. It is advisable to use a jeweler's scale for weighing the sample. If scalp hair is not present, a similar quantity of body hair can be used. When mentioning head hair, we are indicating scalp hair exclusively. Body hair pertains to all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the preliminary handling of a sample within a laboratory's framework. This step includes ensuring the sample was sealed and dispatched correctly, generating a unique LAN (Laboratory Accessioning Number), and executing any supplementary data input absent in an electronic custody chain system.

Screening serves as a primary rapid assessment for drug presence. Though Screening offers a cost-efficient approach to negate drug presence in most samples, any positive screen must undergo confirmation to be legally viable. Samples that test tentatively positive in Screening necessitate further confirmation.

Should a sample show presumptive positivity during Screening, additional hair is extracted from the primary sample and readied for Extraction. In this phase, drug extraction from hair occurs at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug testing the most challenging technique to execute.

Confirmation of positive screening outcomes is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples are cleaned before confirmation if needed. The full laboratory process from Accessioning to Confirmation is overseen under the CAP (College of American Pathologists) Hair designation and compliant with ISO / IEC 17025 standards.
