Yet, when testing for very recent substance use, within the past 5 days, urine tests still offer the highest level of accuracy.

What it is, How it works

Hair testing has become a recognized method for identifying drug and alcohol consumption. It captures long-term evidence of substances by embedding biomarkers within the structure of hair strands as they grow. When hair is acquired close to the scalp, it can reveal alcohol and drug presence for up to about 3 months. The process of collecting hair is straightforward, fairly resistant to tampering, and convenient to transport.

A sample measuring 1.5 inches with roughly 200 hair strands (equivalent to a #2 pencil) near the scalp provides 100mg, which is the optimal amount for examination and confirmation. For EtG, supplements, and/or tests exceeding 10 panels, 150mg is advised. Utilizing a jeweler’s scale to measure the sample is recommended. If scalp hair isn't available, an equivalent quantity of body hair can be used. When the term head hair is utilized, it specifically refers to scalp hair. Body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The drug test result processing in the lab involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of entering a sample into a laboratory’s system, ensuring that the sample is properly sealed and shipped, assigning it a unique LAN (Laboratory Accessioning Number), and completing any additional information not provided by an electronic chain of custody system.

In the Screening phase, an initial rapid check for drug abuse is conducted. While this approach is cost-effective for dismissing drug use in most samples, a positive screen must be verified to be valid in court. Samples that are tentatively positive in Screening require further confirmation.

Should a sample test tentatively positive in the Screening step, additional hair is extracted from the initial collection and prepared for Extraction. This process involves extracting drugs from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening one of the most challenging methodologies to execute.

Verification of any positive Screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS technology. All samples preliminarily positive are washed before confirmation as required. The comprehensive laboratory process, from Accessioning to Confirmation, is reviewed under both the CAP (College of American Pathologists) Hair designation and accredited to ISO/IEC 17025 standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Novel Analytical Techniques: The Verona meeting witnessed the unveiling of cutting-edge analytical techniques, offering enhanced sensitivity, selectivity, and accuracy for drug testing. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
