A sample size of 1.5 inches including around 200 hair strands (comparable to a #2 pencil) nearest to the scalp provides 100mg, which is the optimum quantity for both screening and confirmation. For EtG, additional tests, and/or panels exceeding ten, a specimen weight of 150mg is advised. We suggest using a jeweler’s scale for accuracy. Should scalp hair be inaccessible, a similar amount of body hair is suitable. "Head hair" refers only to scalp hair, whereas "body hair" pertains to all other types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result generally involves four significant phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial introduction of the sample into the lab’s system. This phase ensures the sample has been sealed and transported properly, assigns an arbitrary LAN (Laboratory Accessioning Number), and finalizes any pending data entries absent from the electronic chain of custody system.

Screening constitutes an initial and swift examination for abusive drug presence. While it’s a cost-efficient method to eliminate drug use in most samples, a positive screening requires confirmation to stand valid in legal situations. Any samples deemed presumptively positive in Screening need additional confirmation.

In the event of a presumptive positive screen, additional hair is extracted from the original sample and readied for Extraction. During this procedure, drugs are isolated from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), establishing hair drug testing as one of the most challenging practices.

Confirmation of any positive screening result is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples receive a wash before confirmation when necessary. The entire lab sequence from Accessioning to Confirmation is overseen under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
