What it is, How it works

A hair sample measuring 1.5 inches, consisting of approximately 200 strands (roughly the size of a #2 pencil) closest to the scalp, will yield 100mg of hair, which is ideal for screening and validation. For EtG, additional analyses, and/or tests beyond 10 panels, a quantity of 150mg is suggested. It is advisable to weigh the specimen using a jeweler's scale. If scalp hair is not available, the same quantity of body hair can be gathered. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory process for a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial processing of a specimen into the lab’s system. This step includes ensuring the sample was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening is a preliminary rapid test for drugs of abuse. While Screening is a cost-efficient method to negate drug usage in most samples, a positive screen must be confirmed for it to be legally admissible. Samples that test presumptively positive in Screening require a subsequent confirmation.

If a sample tests presumptively positive in the Screening phase, more hair is extracted from the initial specimen for Preparation. In this phase, drugs are drawn from hair at a concentration significantly lower than other techniques (e.g., urine or oral fluid), which is why hair drug analysis is among the most challenging methodologies to execute.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as required. The entire laboratory procedure from Accessioning to Confirmation is monitored under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO/IEC 17025 standards.
