A 1.5-inch section of roughly 200 strands (approximately the thickness of a #2 pencil) nearest to the scalp yields 100mg, the optimal sample for preliminary testing and confirmation. For EtG, additional tests, or panels exceeding 10, a sample of 150mg is advised. It's prudent to measure the sample using a jeweler’s scale. Should scalp hair not be an option, the same quantity of body hair can be used. "Head hair" denotes only scalp hair. "Body hair" encompasses other types (such as facial and axillary).

Process Overview

Lab processing of drug test outcomes is divided into Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, the sample undergoes initial processing in the lab's system. This step ensures the sample was properly sealed and delivered, allocates a new LAN (Laboratory Accessioning Number), and finalizes any outstanding data entry absent in an electronic custody framework.

Screening provides a preliminary drug of abuse assessment. While cost-effective for excluding drug usage in most samples, a positive screen necessitates confirmation for court validity. Samples showing potential positivity at Screening need further validation.

Upon a presumptive positive in Screening, additional hair is sourced from the original specimen and prepped for Extraction. Here, drugs are extracted at lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug testing the most complex methodology.

Positive screening confirmations employ GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positives are cleansed beforehand as needed. The lab process, from start to finish, is evaluated by CAP (College of American Pathologists) under Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
